Welcome

It is an honor to be part of your journey. Before we begin, please carefully read this Services Agreement (“Services Agreement”) and the Client Acknowledgment, Assumption of Risk, and Release of Liability (“Release”). This Services Agreement will help us to create a foundation of trust, structure, and clarity. I encourage you to reach out to me before our first session if you have any clarifying questions about the Services Agreement or Release; otherwise, please sign both and return to me.

This Services Agreement to provide coaching and consulting services is between the following parties (collectively, “Parties”): Jen Meller LLC (“Practitioner") and Client’s Name (“Client”).

 

Our work together is a collaborative partnership between equals to establish a supportive, thought-provoking, and creative process designed to facilitate the Client’s insight and growth. Our work together is not psychotherapy, medical advice, or mental healthcare, and our work together will not involve the diagnosis, treatment, prevention, or cure of health conditions. 

 

This Services Agreement begins on the date signed by both Parties and continues to be in effect until terminated by either party in writing.

 

Sessions may take place by phone, videoconference, or in person, as agreed mutually between the Client and Practitioner.

Your Responsibilites

  • Set aside distractions (phones, etc.) and be willing to engage and focus on our work together. 

  • Let me know if you feel triggered during a session. It is my deepest desire that you feel seen and supported. If something I say or do (or don’t say or do) causes even a small break in connection, my ask is that you will share this with me so we can work to build a sense of re-connection.

  • If you notice you are starting to disengage from our work together at any point, please let me know. I’m always willing to reassess if this work together is serving you. 

  • You agree that you are responsible for the choices, actions, and results that arise from our partnership.

  • We will work together to identify meaningful goals in our work together. If you want to reassess or shift these goals at any point, please share this desired change with me. I want to support you in a way that helps you move towards what it is you truly want.

My Education & Background

My journey has included formal education in a variety of modalities. I studied at the Spirituality Mind Body Institute at Columbia University, and I’m currently pursuing my doctorate in Clinical Psychology (PsyD), with a focus on Somatic Psychology.

Since 2018, I’ve supported clients across the country and around the world. I bring together the approaches I’m trained and certified in, using them to meet each person where they are and honor their unique process.

To reiterate, it is important to understand that I work with clients as a consultant and coach. My training includes studying alongside therapists in graduate programs and earning certifications in modalities often used by licensed clinicians. While I am currently pursuing a Doctorate in Clinical Psychology (PsyD), I am not a licensed therapist. My role is to support and guide clients—I do not provide medical or clinical treatment, nor do I diagnose, assess, or treat mental health conditions or diseases.

Privacy, Confidentiality & Disclosures

Trust is essential to an effective partnership. Information shared by the Client—whether shared in writing or verbally—will be kept strictly confidential by the Practitioner as a matter of professional ethics with the following limitations: 

 

  • The Practitioner may be legally compelled to disclose client communications and records. The practitioner-client relationship is not a legally privileged one (such as those with an attorney or doctor), so practitioners can be legally required by a court to disclose client communications. 

  • The Client agrees that the Practitioner may disclose client communications if the Practitioner reasonably believes that the Client poses a threat of serious harm to themselves or others.

  • Disclosure of client communications may be made in defense of a complaint or legal claim against the Practitioner.

 

The Client agrees that if the Practitioner provides proprietary materials or techniques to the Client as part of the services, these remain the sole property of the Practitioner. This proprietary information and materials may be used by the Client for personal, non-commercial use and must not be shared or reproduced for commercial purposes.

 

The Practitioner does not voluntarily participate in legal matters, including:

 

  • Testimony, depositions, evaluations, or reports for court

  • Custody or divorce disputes

  • Legal claims or lawsuits

 

If legally subpoenaed, the Practitioner will comply to the extent required by law. Whenever possible, the Client will be notified in advance. Client requests for information to be disclosed to a third party must be requested only through a signed Release of Information in advance.

 

In the event of any dispute involving legal testimony or records, the Client agrees to reimburse the Practitioner for reasonable attorney fees and related costs.

Communications Policy

I am available via text, call, WhatsApp, or email during weekday hours (Monday-Friday, 9am–7pm U.S. eastern time).

Payment & Scheduling

Sessions are $165 per 60 minutes, payable via Zelle or Venmo. (For those who reside outside of the U.S., an alternative method can be agreed upon.)

 

Payment is due at or before the scheduled session.

Cancellation

In the event the Client wishes to cancel or reschedule a booked session, Client agrees to provide at least 24 hours’ notice. If this notice is not provided, Client agrees that payment for the cancelled or missed session is due in full. Rescheduling an appointment is subject to the Practitioner’s availability. Repeated late cancellations or missed sessions may lead to ending our working relationship.

Severability

If any part of this Agreement is found to be unenforceable by a court, the rest of the Agreement shall remain fully in effect.

Jurisdiction; Choice of Law; Venue

This Agreement, and any related matters, shall be governed by, and construed according to, the laws of the State of Arizona without giving effect to conflict of law principles. The parties agree that any legal action or proceeding arising out of or relating to this Services Agreement shall be brought exclusively in the state or federal courts located in Pima County, Arizona, and each Party submits to the jurisdiction and venue of such courts. The parties further waive any objection based on forum non conveniens or any objection to the venue of any such action or proceeding.

Termination & Survivability

Either Party may terminate this Services Agreement at any time by providing written notice. The Client is responsible for payment for all services rendered up to the effective date of termination. If the Client breaches the Services Agreement, or if a Client engages in unsafe conduct, this Services Agreement may be terminated by the Practitioner and no refunds will be provided. 

 

If there is reasonable belief that the Client is no longer benefiting from, or is unlikely to benefit from future services, the Practitioner may terminate this Agreement. The Client will be refunded for any prepaid sessions that were not completed. 

 

The termination or expiration of this Services Agreement shall not affect those provisions that by their nature are intended to survive, including confidentiality obligations, intellectual property rights, jurisdiction and choice of law and venue provisions, non-disparagement, and any payment obligations accrued prior to termination.

Entire Agreement

This Services Agreement, along with the Informed Consent, Assumption of Risk, and Release of Liability, represent the full and complete agreement between the Client and Practitioner regarding the services provided and supersede all prior agreements and communications. Modifications to the Services Agreement or Release must be agreed to in writing by both Parties.

Execution

This Agreement may be signed in multiple copies and can be signed electronically. Together, all signed copies will form one single, binding agreement between the parties.
